The mainstay of TBI management involves admission to the intensive care unit to monitor and minimise additional brain injury caused by increases in intracranial pressure. Some clinicians feel that rises in intercranial pressure may be a late indicator of secondary damage to brain cells, and small trials have suggested that monitoring and optimising the level of oxygen in brain tissue could be effective in minimising secondary injuries. The BONANZA study is a multinational randomised controlled trial comparing standard management of raised intracranial pressure with monitoring and optimising brain tissue oxygen levels in patients who have sustained traumatic brain injury. Functional and neurological outcomes will be assessed at 6 months post injury to determine if there is a difference between the two groups.
